Complete forms in advance when possible. Bring identification, insurance details, medications and recent x-rays. Tell the office if you are pregnant, take blood thinners, need antibiotic premedication, or have significant dental anxiety.
For a child, share previous dental experiences and any sensory or communication needs. That gives the team a chance to plan a calmer introduction.
Set one clear priority
Tell the team what matters most: a routine cleaning, a painful tooth, cosmetic concerns, replacing a missing tooth, or simply establishing care. The first visit can then be structured around the right goal.
If several family members are booking, ask whether appointments can be coordinated and what records are needed for each person.
Leave with a usable plan
Before leaving, you should understand the findings, recommended timing, alternatives and expected costs. Ask which items are urgent, which are preventive, and which can be considered later.
A request form starts the conversation but is not a confirmed appointment until the office contacts you.

Will I receive a cleaning at the first visit?
